HealthPathways has launched in Burlington!
The Burlington Ontario Health Team is thrilled to bring HealthPathways to Canada.
In partnership with the Greater Hamilton Health Network and the Middlesex London Ontario Health Team, the Burlington OHT launched HealthPathways in June 2025, marking a milestone for integrated health systems.
HealthPathways is FREE for all Primary Care Providers in Burlington!
What is HealthPathways?
HealthPathways is a clinician-designed tool that supports healthcare professionals by providing locally relevant, evidence-based guidelines and resources.
Each pathway offers guidance for assessing and managing patients within the local health system and is designed to be used by primary care providers (including family physicians, nurse practitioners and allied health professionals) at the point of care.
As a primary care provider, how will HealthPathways benefit me?
By offering real-time clinical guidance and creating a single access point for healthcare providers, HealthPathways simplifies clinical best practice, local system navigation, and patient education, while empowering healthcare professionals with quick and accurate referral guidance, improving coordinated care and supporting better patient management.
